GP creates a care plan and refers you to allied health services (Physio, OT, Dietitian, psychology). You will need to have the referral made to TeamiCare to access our services under this funding model
Medicare subsidises up to 5 visits per service per year; You will pay the Gap fee.
Plan is reviewed annually to track progress.

- Kitchen & Dining Areas: Lowered or adjustable counters, accessible storage for frequently used items, and adaptive utensils or appliances.
- Assistive Technology & Equipment: Bed rails, hoists, transfer aids, stair lifts or elevators in multi-level facilities, sensor lighting, emergency call systems, communication devices, modified equipment/clothing, wheelchairs or walking aids, and many more.
Allied Health Services in Residential Facilities
- Occupational Therapy: Functional assessments (mobility, self-care, daily living activities), cognitive and memory support programs, assistive technology and equipment recommendations, home modifications within the facility (bedroom, bathroom, communal areas), and falls prevention programs.
- Physiotherapy: Mobility and balance assessments, exercise programs to maintain strength, flexibility, and endurance, pain management (manual therapy, exercise, posture correction), falls prevention and safe transfer training, and respiratory therapy and postural management.
- Dietitian: Nutritional assessments and care plans, meal planning for specific health conditions (diabetes, dysphagia, renal issues), weight management and supplementation guidance, and monitoring hydration and malnutrition risk.
